During the school year our School Office hours are 7:30 a.m. until 3:15 p.m. 

Before a student can be considered for enrollment the following items MUST be received
(the forms constitute the Registration Packet):

m       Family/Student Registration Form

m       New Student Data Form (Social Security Number must be listed or registration will not be processed) 

m       Tuition Payment Commitment Form   

m       Deposit Policy Form 

m       Principal/Director Recommendation Form   (give to CURRENT school for processing)

m       New Student Data Form (Social Security Number must be listed or registration will not be processed)   

m       Survey for Parents of Incoming Students  

m       Deposit Check, payable to "IHM School"

m       Release of Records Form (this form must be returned to OUR School Office so your child’s current school can transfer the official transcript of grades and medical records.  This Release Form will not be mailed until the withdrawal date from your child's current school)

m        Copy of Birth Certificate

m      If transferring from another Catholic or private school, attach a personal letter stating that your financial obligations to the school are current.

m       Copy of Baptismal Certificate (unless baptized at Immaculate Heart of Mary Parish)

m       Letter of good standing from a former parish (if Immaculate Heart of Mary parishioner less than six months)

m       Copy of court order granting custody (in cases of divorce, adoption, foster parents or other court-ordered custody)

m       Students entering grades 2-8: Copies of current and previous standardized testing and report cards (current year and past two years)

m       Copies of psychological testing, current IEP or ISP, or medical testing which may relate to student’s learning, or success in school.

 REGISTRATION DEADLINES FOR 2008/2009

INCOMING FIRST GRADERS for 2008/2009
A readiness assessment must be given to every child entering our first grade. You will be notified if the readiness assessment determines your child is not ready for first grade.  The assessment dates are March 31, April 3, April 4.  The assessment includes language development, auditory perception, vision and hearing screening.  Call Lisa Day at 388-3021 to schedule a date for your child’s assessment.  Incoming first graders have a limited schedule the first week of school and more information concerning this schedule will be sent at a later date.

 UNIFORMS FOR 2008/2009

·      Schoolbelle's Uniform Company, 921-3417, 9695 Kenwood Road or  www.schoolbelles.com     Our school code is 1924   Schoolbelle's will host a Measuring Night at our school in Heritage Hall on Thursday, May 15, from 6:30-8:30 p.m.

·      Our PTO manages a Used Uniform Program.  This is your opportunity to purchase used polo shirts, skirts, sweatshirts and pants. In September and May they host a Used Uniform Sale at our school. The next sale will be Tuesday, May 20, from 6:30-7:30 p.m. in Heritage Hall.  CASH ONLY FOR THE USED UNIFORM ITEMS (CHECKS WILL NOT BE ACCEPTED)!  Bring singles as change will not be available.
