The Preschool Child Medical Statement and a copy of your child's immunizations must be kept on file in the Preschool Office. The policy for medical records/immunization from The Ohio Department of Education guidelines state ... (A) The parent shall provide, prior to the date of admission or not later than 30 days after date of admission and every 13 months from the date of examination thereafter, a report from a licensed physician affirming that the child is in suitable condition for enrollment in the program. “Prior to the date of admission” means: For children 3 years old or older at the time of admission, the examination shall occur within 12 months prior to the date of admission. All students with medical conditions, allergies or medications needed during the school day must complete the necessary forms and return to the Preschool Office to be kept on file at all times.
